Notice about Signature Requirements for Application Form of Legalization and Declaration Form of Legalization

 

According to the requirements from the Embassy of the People's Republic of China in Malaysia, the signature of applicant at Part 8 of the Application Form of Legalization and the Declaration Form of Legalization must be signed by the applicant himself or herself when the applicant applies for legalization. Anyone else signing the forms on behalf of the applicant is not allowed. If the application is submitted by the agency (or third party), the scanned, faxed, photographed, printed or photocopied form is not accepted. The applicant may deliver the application form signed by the applicant with the date of completion to the agent or third party by courier or other methods.

 

If the applicant’s signature cannot be verified by the documents submitted, a copy of the applicant's passport information page and signature page is required.


    If the signature in the application form or the Declaration Form of Legalization is inconsistent with the applicant's signature or does not meet the above requirements, the Kuala Lumpur and Penang Chinese Visa Application Service Center will not accept the application. All consequences will be borne by the applicant and agent.
